Autism Survey

You can help identify risk factors for Autism Spectrum Disorders by taking an online survey. 

To access the survey, please click on the link at the bottom of this page.

You will be asked to confirm that you are eligible to participate.  To participate you must be a biological mother and at least 18 years old on the day you take the survey.

You will also be asked to review information about confidentiality and the purpose of this study and to decide if you understand that information and wish to participate.

If you confirm your eligibility and choose to participate, you will be directed to the rest of the survey.  We will ask questions about your family’s health in general and, in particular, for the time during and after each of your pregnancies.
