MEDICATIONS

ANTIBIOTICS
SBE prophylaxis
  • Ampicillin 2g IV AND Gentamicin 80mg at 8-9 cm dilated, then repeat Amp AND Gentamicin 8° after delivery. 
  • PCN allergy – use Vancomycin 1g IV and Gent as above
GBS prophylaxis
  • Ampicillin 2g IV load then 1g Q4°
  • PCN allergy – Clindamycin 900mg IV q8°
Chorio
  • Intrapartum Unasyn 3g IV Q6h, if C/S Gentamicin (2 mg/kg load, then 1.5mg/kg Q8h) AND Clindamycin 900mg Q 8h
Endometritis
  • if SVD then Unasyn 3g IV Q6h, if C/S, Gentamicin 2 mg/kg load, then 1.5mg/kg IV Q8h AND Clindamycin 900mg IVQ8h
BV
  • Flagyl 500mg po bid x 7 days/MetroGel to vagina bid x 5 days (not in first trimester)
Trichomonas
  • Flagyl 2g po x 1
GC
  • Rocephin 250 mg IM x 1 and Zithromax 1g po x1 (NO Doxy if pregnant)
Chlamydia
  • if pregnant or no BC, Zithromax 1g po x 1
  • if not pregnant, Doxycycline 100mg po bid x 7d
UTI’s
  • Macrobid (nitrofurantoin) 100mg bid x 7 days
  • TMP/SX DS bid x 7 days (not first trimester or near term)
PPROM
  • Ampicillin 4g IV load then 2g Q4°
PAIN MEDICATIONS
Demerol
  • 50–100 mg  IV q3-4° / Phenergan 25 mg IV q6°
Morphine
  • 2-4 mg IV q1-2° or 10 mg IM q3-4°
Fentanyl
  • 100 µg IV q1°
Epidural
  • may have in active phase – some say when 4 cm dilated or on Pitocin
Vicodin
  • 1-2 tabs po Q4-6h prn pain
OTHER MEDICATIONS
N/V
  • Reglan 10 mg po QAC and QHS
  • Phenergan 12.5 or 25 mg po/IV/PR Q 4-6 h prn
  • Zofran 8mg po BID
Constipation
  • Surfak 240 mg po QHS
  • MOM 30ml po Q6h until BM up to 4 doses
  • Dulcolax 10mg po/pr QHS
Fever
  • Tylenol 650mg po Q4-6h prn (after thorough work up)
